Advanced Nurse Practitioner - Community / Frailty & Minor Illness

Location: Isle of WightSalary: £50,000 - £60,000 DOEHours: Full-time, Monday-Friday

Are you an ANP with experience in community nursing, frailty, and minor illness management? Do you want a rewarding role preventing hospital admissions through proactive patient care? This is the role for you.

What you'll do:

• Assess and manage minor illness and frailty-related conditions
• Conduct home visits to support patients in the community
• Focus on proactive care and admission prevention
• Work as part of a Neighbourhood / NDT nursing team
• Receive GP supervision and support
• Be employed via a Good CQC-rated training GP practice

You'll need:

• Experience in community nursing, frailty, and minor illness
• Mobility & access to a car for home visits
• Prescribing experience is desirable but not essential
