Job Description

We're a General Practice that genuinely puts patients front and centre to all that we do everyday.

Main duties of the job

This is an opportunity for a fixed term contract to cover maternity leave for 12 months.

We will need initially 4-6 clinical sessions and then from mid autumn an additional 6-7 sessions too.

The role is in-person, at the surgery for each session. In accordance with the rota you will undertake a variety of GP duties inc both face to face and telephone consultations, a share of the duty doctor cover, online consults (via accuRx and document management (via DOCMAN)

Job responsibilities

JOB SUMMARY

The post-holder will manage a patient caseloadand deal with a wide range of health needs in a primary care setting, ensuringthe highest standards of care for all registered patients. Responsibilitieswill reflect requirements of the NHS GP Contracts, and as such may be subjectto change as services adapt.

In accordance with the practice rota, thepost-holder will undertake a variety of duties including face to face and telephoneconsultations, as well as a share of duty doctor cover, online consultations - via Accurx, and document management - via Docman.

You will be required to work as an autonomouspractitioner, responsible for the provision of medical services to the diversepractice population, delivering an excellent standard of clinical care whilstcomplying with the GMS contract and GMC standards for good medical practice.

MAIN DUTIES & RESPONSIBILITIES

Clinical responsibilities:

  • Undertakingface to face, telephone and online consultations and queries, visiting patientsat home
  • Generating,checking and signing repeat prescriptions and dealing with queries, results,tasks, and correspondence in a timely fashion
  • Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation
  • Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems
  • Screeningpatients for disease risk factors and early signs of illness
  • Inconsultation with patients and in line with current practice disease managementprotocols, developing care plans for health
  • Providingcounselling and health education
  • Managingpatients within the wider MDT
  • Admittingor discharging patients to and from the caseload and referring to other careproviders as appropriate
  • Recordingclear and contemporaneous consultation notes to agreed standards
  • Collectingdata for audit purposes
  • Prescribingin accordance with the practice prescribing formulary or generically wheneverthis is clinically appropriate
  • Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.
  • Completionof private referrals and NHS letters
  • Activelyengaging in maximising the QoF points for the practice at each contact withpatients when appropriate.
  • Contributingto the effective management of the practice, leading by example, maintaining apositive, collaborative working relationship with the multidisciplinary team.

  • Contributingto the summarising of patient records and read-coding patient data

Person Specification Qualifications
  • GMC Registered, not subject to
  • suspension
  • National performers list registration
  • Clear enhanced DBS check
  • Evidence of continued professional development
  • Eligibility to practice in the UK
  • DFSRFH & competence in coil insertion
Experience
  • Experience of working in UK General Practice
  • Experience of working with vulnerable groups and an understanding of how to support them
  • Understanding of the NHS system and challenges facing the NHS
  • Experience of clinical audit delivery
  • Experience/understanding of QOF management
  • Experience using EMIS Web, Accurx and Docman
  • Understanding of GMS contract and Enhanced Services
  • Understanding of PCN Networks/GP Federations
