Personal Advisor

We have a permanent 3.5 day a week opportunity available to work as a personal advisor within the transition to adulthood service.
This is an exciting opportunity to support our care experienced young people aged 16-25 as they move towards independence. We are looking for a dynamic individual who is passionate about supporting young people and has a good understanding of the needs and barriers our young people face.
As a Personal Advisor, you will:- Work alongside qualified Social Workers to assess needs and plan for independence
- Hold a caseload of care leavers aged 18+, including those with complex needs such as mental health challenges, offending history, or immigration matters
- Coordinate, implement and review Pathway Plans, ensuring every young person has a clear and realistic route to adulthood
- Attend statutory meetings such as Looked After Reviews and Personal Education Plans
- Advocate for young people and empower them to make informed decisions about housing, education, employment and health
- Provide practical, hands on support including help with moving, accessing benefits, and setting up a first home
- Build strong multi agency partnerships to deliver coordinated, high quality support
- Maintain accurate case records and monitor key outcomes including education, employment and immigration status
- Participate in a duty rota and work flexibly, including occasional out of hours and lone working

- Work effectively in partnership with statutory and voluntary agencies

- Are organised, reflective and committed to continuous improvement
You will be part of a dedicated Team focused on improving outcomes, strengthening multi professional working and putting young people at the centre of decision making. Regular supervision, training and development opportunities are provided, including safeguarding and legislative training.
