Hospital Discharge Senior Case Worker

Senior Case Worker role based at Winchester Hospital, Hampshire County Council, is a permanent post within our social care and health care team. You'll assess and review individuals' care needs across hospitals and community settings, supporting safe hospital discharges and coordinating with families and professionals.

You will work to maximise wellbeing and independence using a strengths-based approach, collaborating across multidisciplinary teams.
