- Act as the Lead Professional for children, young people, and families requiring targeted Early Help intervention.
- Undertake strength-based assessments and identify risks, protective factors, and support needs using recognised assessment frameworks.
- Develop, implement, and regularly review SMART family support plans to achieve agreed outcomes.
- Coordinate and chair Team Around the Family (TAF) meetings, ensuring effective multi-agency collaboration.
- NVQ Level 4 or above in a relevant field such as Health and Social Care, Early Years, Youth Work, Education, Social Care or equivalent professional qualification.
- Relevant safeguarding and child protection training.
- Evidence of continuing professional development.
- Minimum of three years' experience working with children, young people, and families.
- Experience managing complex caseloads within Early Help, family support, youth justice, education, social care or related services.
