Join our dedicated team delivering intravenous therapies within patients' homes across Plymouth, South Hams, and West Devon. This part time role (up to 24 hours per week) is ideal for an experienced, motivated nurse who is eager to contribute to a growing Urgent Care Nursing service.
As a community nurse within the Intravenous Therapy Day Service, you will provide high quality, patient centred care, helping individuals remain safe, independent, and supportive of their health and wellbeing.

Job Responsibilities• Deliver evidence based care for acutely ill patients at home, including intravenous therapies, medication administration, wound care, and line management (TIVAD, PICC, Midline).
• Assess, plan, implement, monitor and evaluate patient care autonomously and as part of a multidisciplinary team.
• Coordinate care and act as a patient advocate, facilitating appropriate care pathways with hospital, primary care, community and social care services.
• Work within challenging, unpredictable environments and maintain accurate, confidential records compliant with NMC guidelines and local policies.
• Manage infection control, emergency situations (e.g., anaphylaxis, cardiac/respiratory arrest, sepsis), and acute observations (NEWS 2, ABCDE). Perform investigations such as ECG, blood glucose monitoring, and blood tests.
• Maintain the standard of patient records by audit or peer review, and support the team manager in investigations of incidents and reporting.
• Mentor pre registration nursing students and support the learning environment for the team.
Person Specification - Knowledge, Skills & Experience- Registered Nurse (Adult) with current NMC registration and relevant post registration courses.
- Strong background in acute community care, intravenous therapy, medication management and line insertion.
- Comprehensive understanding of Clinical Governance, NMC Code, and acute care guidelines.
- Demonstrated autonomous practice, risk management, and multidisciplinary teamwork.
- Experience with emergency care management, infection control, palliative/end of life care.
- Strong organisational and time management skills and ability to travel independently across the service area.
- Excellent communication skills with patients, carers, and professional colleagues.
- Commitment to evidence based practice, continuous professional development, and quality improvement.
- CNP (Community Nursing Practice) or equivalent training.
- Mentoring or teaching qualification at degree level or equivalent.
- Non medical prescribing qualification (preferred).
- Clinical assessment and management in community settings.
- Experience working in multiple health settings and managing diverse patient needs.
£32,073 to £39,043 per annum (pro rate).
