The Evesham, Broadway, Bredon and Inkberrow (EBBI) Neighbourhood Team is a friendly and hardworking team who are looking for an enthusiastic nurse to join us at our base in Evesham.

The Neighbourhood team is looking to recruit a community nurse who is happy to work across our whole patch 37.5 hours per week and keen to join the team in supporting us to deliver high quality care.

We are looking for a passionate nurse who is excited by change and keen to help shape a new way of delivering community nursing care. We work closely with our GPs and work hard at developing relationships with primary care services, drawing on the skills of our MDT colleagues to manage complex patients and ensure we are responsive, and patient centred at all times.

Main duties of the job

Rolerequirements:

  • Working flexibly across the local area (the service operates 3 shifts within 7:30am-6.30pm, Mon-Sun)
  • The ability to manage your own caseload and work within an MDT
  • The ability to problem-solve and manage time effectively
  • Experience of working in a fast paced and varied environment

Your typical day will include home visits for adults of 18+ needing a range of nursing interventions. These include end of life care, and pain/symptom management. Wound care: post-op wounds, leg ulcers, pressure ulcers and complex dressing regimes. Other skills required will be in IV therapy, injectable drugs, and holistic patient assessments. You will need to cover clinic patients and respond to unplanned and 2-hour request visits along with those from UCR.

Back at base, you will be involved in team meetings and care planning. You may also support the teaching of nurse students pre and post registration.

Job responsibilities

The post holder will:

  • Work as an autonomous practitioner within the integrated team providing rapid and planned assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands on clinical skills and knowledge to treat, educate, and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care.
  • Regularly take responsibility for the caseload in the absence of the Senior Community Nurse.

Person Specification Knowledge
  • Variety of evidence based clinical skills
  • Confidence to teach student nurses, trainee nursing associates, patients, carers, and members of the multidisciplinary team
  • Understanding of evidence-based nursing, multidisciplinary working, clinical governance
  • Competence in male and suprapubic catheterisation
  • Venepuncture and IV therapy
  • Cannulation
  • Management of syringe drivers
  • Wound and Palliative care
Qualifications
  • Registered Nurse (RN1)
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
  • Accredited courses in palliative care, tissue viability, Diabetes
Skills and Abilities
  • Ability to challenge poor behaviour
  • Role modelling good behaviour that supports teamworking
  • Ability to communicate effectively and clearly in English in both written and verbal formats
  • Ability to understand professional and ethical issues
Additional Criteria
  • Able to meet the travel requirements for the role
Experience
  • Experience gained during Pre-Registration training
  • 2 years post registration experience
  • Community nursing experience
  • Experience of management of caseloads
