Community Mental Health Nurse

What You'll Be DoingComprehensive Care: You'll lead systematic nursing assessments that consider the "whole person" including their culture, environment, and psychology.Collaborative Planning: Working side by side with service users, their carers, and a multi disciplinary team, you'll co create person centered care plans that actually mean something to the individual.Clinical Excellence: You'll act as a mentor and leader, ensuring our practice stays in line with NICE guidance while delivering hands on care and evaluation.Service Evolution: From facilitating Depot and Clozapine clinics to driving Quality Improvement (QI) initiatives, you'll help us constantly find better ways to serve our community.
What We're Looking ForA Holistic Thinker: Someone who understands that health is influenced by everything from social factors to spiritual needs.A Natural Leader: You're comfortable guiding best practices and supporting your teammates.The Essentials: Valid nursing registration and a solid understanding of NICE guidelines and multi agency working.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010). Main duties of the job- Supporting patients to function to their optimal ability in their environment.
- Maintaining and promoting a safe environment taking account of infection control and identifying and reporting hazards and risks in a timely manner
- Escorting and transporting patients as required as part of their care plan e.g ECT Suite.
- May also be required, at times, to work at other Trust units and/or sites.
- Physically able to carry out all duties required of the care setting, following successful completion of physical interventions training as required.

Person Specification Training,Qualifications and Registration.- Registered Mental Health Nurse with NMC
- Practice Assessor
- Completed Preceptorship
- Completed Edward Jenner Programme
- Evidence of continuing professional development within mental health field.
- Significant experience at Band 5
- Evidence of post qualifying continued learning
- Experience of working within the NHS.
