An exciting opportunity has arisen for a Community Assistant Practitioner to join the Helensburgh & Lochside District Nursing Team. We are looking for a compassionate, motivated and enthusiastic practitioner to join our friendly and supportive community nursing team. This rewarding role supports the delivery of high-quality, person-centred care within patients' homes, across the Helensburgh & Lochside area.
About the RoleYou will work as part of an integrated multidisciplinary team supporting individuals with a wide range of complex and long-term health needs within the community. Working under the delegation of a Registered Nurse and within your defined scope of practice, you will:
- Support clinical assessment, planning, implementation and evaluation of care
- Deliver delegated clinical interventions including wound management, catheter care, venepuncture, blood glucose monitoring and vital signs
- Recognise and escalate deterioration or clinical concerns appropriately
- Plan and prioritise delegated home visits with minimal supervision
- Maintain accurate written and electronic patient records
- Support Healthcare Support Workers in partnership with the Registered Nurse
- Contribute to audit, quality improvement and clinical governance
This role involves lone working, regular travel and working in varied community environments.
Who We're Looking ForWe're looking for someone who brings both compassion and competence to community practice. You will have:
- Experience working within the community
- SCQF Level 7 (e.g. SVQ 3 in Healthcare or equivalent) and/or SCQF Level 8 (e.g. PDA or equivalent experience - or equivalent demonstrable practical experience)
- Experience of working with delegated responsibilities and minimal supervision
- The ability to work independently and as part of a multidisciplinary team
- Strong communication, organisational and problem-solving skills
- IT literacy and confidence using electronic systems
- A full current driving licence and access to a car for work purposes

ADDITIONAL INFORMATION FOR CANDIDATES- Effective from 1st April 2026, the Agenda for Change full-time working week will be reduced from 37 to 36 hours. Part-time hours will be adjusted on a pro-rata basis. There will be an increase in the hourly rate to ensure that pay remains unaffected.
