Main duties of the jobThe Clinical Nurse Manager will lead the operational management of the Virtual Ward and Acute Hospital at Home team in North to ensure that: high standards of care delivery are achieved and maintained; the service is delivered in an efficient and cost-effective way within allocated resources; and the service is evolving to keep up with best practice and digital innovations. The post holder will be based in North Devon District Hospital and may be required to work in other areas as directed by the line manager.
The post holder will work as part of a team, in partnership with the Clinical Nurse Manager in Eastern and the Lead Nurse for Acute Hospital at Home, as well as with community colleagues. In addition to management responsibilities, the post holder will work clinically within the service; therefore a strong background in acute medicine or other acute nursing is required.

An exciting opportunity has arisen within Acute Medicine for a dedicated, enthusiastic and experienced Registered Nurse to lead our integrated Acute Hospital at Home team in North Devon, working closely with the Lead Nurse and the Band 7 Clinical Nurse Manager in our Eastern services.
Acute Hospital at Home is a safe and efficient alternative to NHS bedded care. It is a Virtual Ward, which supports patients who would otherwise be in hospital to receive acute care at home, including preventing avoidable admissions or supporting early discharge.
The post holder will:
- Function as a professional role model and clinical leader, demonstrating clinical and managerial competence.
- Be highly visible and accessible in clinical areas to facilitate communication with staff and patients, observe practice and ensure standards are maintained.
- Promote and support innovative practice and ensure research findings are incorporated into nursing practice, including creating and developing new pathways and regularly reviewing those in use.
- Work to improve and maintain the AHAH service based on best practice and current national guidelines.
- Be involved with the development and management of Acute Medicine Ambulatory Care in North, setting up the service and ensuring it runs smoothly.
- Lead delivery of digitally enabled care (virtual care) including virtual wards as part of a multi-disciplinary team to drive transformation and improve patient outcomes.
The successful candidate must have excellent communication skills and work well as part of a multi-disciplinary team. You must be able to work under pressure and be committed to leadership development. A priority is service development and ongoing expansion of the Virtual Ward. As we move toward integrated care, dynamic teamwork is vital.
Working Pattern: Day shifts, 4 days a week. May include weekends.
