Care Team Leader - Health & Social Care

Praxis Care is seeking a Team Leader to guide a group of Support Workers in delivering person-centred care at Castle Lane. You will design individual support plans, coordinate services, and ensure safety, dignity and independence for people we support.

The role requires leadership, strong communication, and organisational skills with a focus on regulatory compliance and ongoing development opportunities for staff. Sleep-ins may apply with additional pay where required.
