Are you a dynamic, innovative, and enthusiastic Advanced Clinical Practitioner with a passion for supporting individuals with memory difficulties? Do you want to be part of a pioneering initiative that is reshaping Dementia and Frailty services across Birmingham?
We are excited to offer a unique opportunity to join a newly funded project under the Birmingham Fairer Future Fund, aimed at tackling health inequalities through culturally sensitive and collaborative care pathways. This role will be central to the co-design and delivery of new assessment models, working closely with system partners and community stakeholders to improve awareness and access-especially for under-represented groups.

Job responsibilitiesFor further information about the main responsibilities, please view the attached job description and person specification.
Person Specification- Current registration with NMC, HCPC, etc.
- MSc (Level 7) in accredited Advanced Clinical Practice.
- Independent non-medical prescriber at Level 7.
- Leadership skills: ability to engage and deliver change through partnership working.
- Extensive post-registration experience with older adults, memory services, and frailty.
- Ability to develop and sustain partnership working across individuals and organizations.
- Understanding of primary and secondary care integrated service models.
- Commitment to improving co-production and patient and carer experience and outcomes.
- Contribution to pathway redesign and post-diagnostic support.
