Main duties of the jobTo deliver a high standard of patient care using advanced autonomous clinical skills and a broad theoretical knowledge base.
Working in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.
To manage own clinical workload in general practice, working within their own professional boundaries, responding effectively to patient need.

Job responsibilitiesProvideadvanced clinical assessment, diagnosis, and treatment for patients with acuteand chronic conditions
Prescribemedications and manage care plans independently
Support urgent care and same-day appointments
Home visits
Support the GPs by carrying out ward rounds to our local care homes
Contributeto QOF, audits, and clinical governance
Workclosely with GPs and the wider team to improve patient outcomes
Participatein safeguarding, risk management, and practice development
Person Specification Qualifications- Registered Nurse with active NMC reistration
- MSc in Advanced Clinical Practice or equivalent qualification
- Full UK driving licence
- Experience of working in Primary Care
- Experience with EMIS
