Job Description

Are you an experienced Advanced Nurse Practitioner looking tomake a real impact in primary care? Due to retirement we are seeking a dynamic, compassionate ANPto join our multidisciplinary team and help us deliver high-quality,patient-centred care.

Main duties of the job

To deliver a high standard of patient care using advanced autonomous clinical skills and a broad theoretical knowledge base.

Working in collaboration with the GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.

To manage own clinical workload in general practice, working within their own professional boundaries, responding effectively to patient need.

Job responsibilities

Provideadvanced clinical assessment, diagnosis, and treatment for patients with acuteand chronic conditions

Prescribemedications and manage care plans independently

Support urgent care and same-day appointments

Home visits

Support the GPs by carrying out ward rounds to our local care homes

Contributeto QOF, audits, and clinical governance

Workclosely with GPs and the wider team to improve patient outcomes

Participatein safeguarding, risk management, and practice development

Person Specification Qualifications
  • Registered Nurse with active NMC reistration
  • MSc in Advanced Clinical Practice or equivalent qualification
  • Full UK driving licence
Experience
  • Experience of working in Primary Care
  • Experience with EMIS
