Youth Transition & Independence Coach

Horizon Care and Education in Derby is seeking a Youth Support Worker to help young people move towards independent living. The role includes emotional support, developing plans, and facilitating access to education and employment.

You will work with vulnerable young people, ensuring safeguarding and risk management while collaborating with professionals. A driving license is desirable and shifts include evenings, weekends, and sleep-ins.
