Job Description

Wyre Forest Network of Independent Practices is a dynamic and forward-thinking Primary Care Network, committed to delivering high-quality, patient-centred care. We are looking for an experienced and motivated Frailty ACP to join our expanding multidisciplinary team, supporting our population of older adults across care homes and the wider community. We will consider part time for the right candidate.

Main duties of the job

As a Visiting ACP you will provide advanced clinical care and proactive management of residents in care homes and frail patients in their own homes. This is a patient-facing role with a strong focus on personalised care, frailty assessments, and medicines optimisation. You will work closely with GPs, pharmacists, care coordinators, and other allied health professionals within the PCN. The aim to prevent avoidable hospital admissions, manage complex health needs and deliver holistic care.

Wyre Forest Network of Independent Practices (WFNIP) PCN are a Primary Care Network serving a population of 44,000 over 4 practices and consider themselves as a forward thinking, innovative and driven team of professionals meeting the demands of patient care. We have a diversified workforce made up of GPs, ACP, Nurses, Clinical Pharmacists, First Contact Physiotherapists, Pharmacy Technicians, Health Care Assistants, Social Prescribers, Mental Health Practitioner and Care Coordinators.

Job responsibilities

Conduct comprehensive frailty assessments and create personalised care plans.

Undertake regular care home rounds and domiciliary visits.

Prescribe and review medications in line with clinical guidelines and patient needs.

Lead on anticipatory care planning and end of life discussions.

Work collaboratively to reduce avoidable hospital admissions.

Support MDT meetings and contribute to care home alignment projects.

Deliver holistic care with a focus on independence, dignity, and quality of life.

Ability to work autonomously and with the MDT to manage acute and chronic conditions.

Person Specification Qualifications
  • Registered Practitioner with a valid registration
  • Experience in frailty, community care delivery of the elderly, or care home settings.
  • Confident in lone working, decision-making, and working across care interfaces.
  • Knowledge of Frailty assessment tools (e.g Rockwood, eFI) and personalised care principles
  • Knowledge of Palliative Care Delivery.
