We are seeking a highly skilled and compassionate Triage Nurse to join our dynamic and inclusive healthcare team. The Triage Nurse will play a pivotal role in providing critical first contact with patients, ensuring they receive appropriate care with efficiency and empathy. Responsibilities
  • Assess and prioritize patients' health concerns via phone, in-person, or online communication, ensuring timely and appropriate care.
  • Utilize evidence-based practice to guide clinical decision-making and provide recommendations for next steps in care.
  • Collaborate with physicians, specialists, and other healthcare professionals to ensure seamless care delivery and accurate information transfer.
  • Document patient interactions accurately and thoroughly in electronic health records (EHR) following institutional and regulatory guidelines.
  • Provide education and support to patients and families to help them better understand their condition and care options.
  • Monitor patient follow-ups and track progress to minimize risks and improve health outcomes.
  • Participate in ongoing professional development and maintain current knowledge of industry best practices, policies, and standards.
  • Advocate for diversity, equality, and inclusion within the workplace and in patient care delivery.
Qualifications
  • Active and unencumbered Registered Nurse (RN) license in Insert location/state as per role .
  • Bachelor's degree in Nursing (preferred) or Associate's degree in Nursing.
  • Minimum of 2 years of nursing experience, with at least 1 year in a triage, emergency, or acute care setting.
  • Strong clinical assessment, critical thinking, and decision-making skills.
  • Exceptional communication and interpersonal skills to convey empathy and effectively address patient concerns.
  • Proficiency with electronic health records (EHR) and other healthcare technology systems.
  • Ability to work in a fast-paced environment while maintaining attention to detail.
  • Commitment to providing equitable care to patients from diverse cultural and socioeconomic backgrounds.
Day-to-day
  • Begin your day with a team huddle to align on patient priorities and overall care goals.
  • Triage incoming calls or patient visits, determining the urgency of care and making recommendations for treatment or resources.
  • Communicate with patients about symptoms, treatments, and care instructions in a respectful and culturally sensitive manner.
  • Update patient records in real time to ensure that the care team has accurate, up-to-date information.
  • Provide emotional support and reassurance to patients and families dealing with medical uncertainties.
  • Attend training, workshops, or team meetings focused on professional development and enhancing cultural competency in patient care.
  • Work collaboratively with multidisciplinary healthcare professionals to coordinate patient care effectively.
  • End the day by reviewing unresolved patient concerns and ensuring follow-up as needed.
