Job Description
Pembrokeshire County Council is recruiting a Team Leader for the Community Wellbeing and Independence Team in South Pembrokeshire. You will lead a dedicated team delivering support to individuals living in their own homes, coordinating daily living activities, personal care, meal preparation and medication administration.
You will write and review care plans, complete risk assessments, and ensure standards meet Social Care Wales and Care Inspectorate Wales requirements.
