Team Leader - Targeted Care and Enablement Service

We are looking for a motivated and experienced Team Leader to join our Targeted Care and Enablement Service - a short term, responsive service that plays a critical role in supporting people to regain skills, remain independent at home, and avoid unnecessary hospital admissions.

This is a dynamic and rewarding leadership role where you will lead a patch based team delivering enablement, bridging, and fast response care services across Ceredigion.

We are seeking someone with strong partnership, communication and analytical skills, and experience in social care commissioning or service coordination. In return, you will join a supportive, forward thinking council that values innovation, flexibility and community impact.
