• Individual Outcome and Wellbeing: Implement person centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
• Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.
• Health and Lifestyle: Support individuals in maintaining healthy lifestyles and managing their health needs.
• Communication: Facilitate effective communication and maintain detailed, person centered records.
• Medication Management: Oversee medication processes and support individuals in self administration.
• Safety and Environment: Maintain a safe, clean living environment and adhere to health and safety protocols.
