Job Description

City Health Care Partnership CIC is seeking an Associate Practitioner to support stroke rehabilitation and enhance independence and wellbeing across patients' home and community settings.

You will work within a friendly, skilled team coordinating with Physiotherapy, Occupational Therapy, Speech and Language Therapy, and Nursing colleagues; typical hours are 08:00-16:00, with some condensed hours and weekend work to enable flexibility.
