The Band 5 Staff Nurse is responsible for delivering high-quality, patient-centred nursing care to adults undergoing specialist neurorehabilitation. Working as part of the multidisciplinary team, the post holder will assess, plan, implement and evaluate individualised rehabilitation programmes for patients with complex neurological conditions, promoting independence and maximising functional recovery.

The role involves caring for a defined group of patients, monitoring neurological status, administering medications safely, and recognising and escalating changes in clinical condition appropriately. The Band 5 Nurse will contribute to goal setting processes, discharge planning and collaborative working with therapy teams, medical staff and community services to ensure safe and effective patient pathways.

The post holder will support healthcare support workers and students, participate in quality improvement and audit activities, and maintain professional development in line with the NMC Code and Trust policies. They will act as a positive role model, uphold high standards of documentation and infection prevention and control, and contribute to maintaining a safe, compassionate and therapeutic rehabilitation environment.

Please note this role requires NMC Registration.

Main duties of the job
  • Deliver safe, effective and compassionate nursing care to patients with complex neurological conditions within a specialist neurorehabilitation setting.
  • Assess, plan, implement and evaluate individualised, goal focused rehabilitation care in collaboration with the multidisciplinary team (MDT).
  • Monitor patients' neurological and physical condition, recognising and appropriately escalating any deterioration or concerns.
  • Administer medications safely and in accordance with Trust policy and the NMC Code.
  • Contribute to rehabilitation goal setting, discharge planning and patient flow in partnership with therapy teams and community services.
  • Maintain accurate, timely and confidential patient documentation, including neurological observations, care plans and risk assessments.
  • Promote patient advocacy, dignity and independence, supporting patients and families throughout the rehabilitation journey.
  • Supervise and support healthcare support workers and students as appropriate to role and experience.
  • Adhere to infection prevention and control standards, health and safety policies, and risk management procedures.
  • Participate in audit, quality improvement and ongoing professional development to maintain high standards of neurorehabilitation practice.

This service has a planned move from Poole site to Bournemouth site on January 2027.

If a role or service relocates as part of a planned move, excess mileage will not be reimbursed. Travel from home to the new work base will be classed as a normal commute. Any other changes will be managed under Trust or national terms and conditions.

Person Specification Qualifications
  • NMC Registered Nurse
  • Post registration courses related to the speciality
Experience
  • Experience of caring for patients with acute and long term conditions
  • Demonstrate clinical experience
  • Previous experience in Neurorehabilitation
  • Experience of end of life care
Skills
  • Demonstrate ability with vital signs monitoring equipment and ability to recognise the deteriorating patient
  • Experience in complying with nursing, infection control and risk management polices
  • Ability to manage caseload/bay of patients
  • Non IV and IV medication administration including administration of blood products
  • Able to integrate and work within the MDT with effective communication skills
  • Experience of clinical skills such as: ECG recording, CVC, Cannulation and Venepuncture, Male catheterisation
  • Specific skills relevant to specialty eg. Caring for patients with spinal cord injury
Knowledge
  • Knowledge of conditions relevant to speciality
  • Ability to demonstrate fundamental nursing knowledge and practice skills
  • Awareness of patient experience, quality and safety agenda's that support Governance Framework
