Social Worker

As a Social Worker at Farmfield Hospital in Charlwood, Surrey, you will be part of a multidisciplinary team providing care for men and women with complex mental health needs. You will manage a complex caseload, conduct assessments, facilitate group activities, and contribute to care planning to support service users in making plans for their future. Main duties of the job

As a Social Worker, you will be responsible for referrals to LADO and Local Authority in line with safeguarding, educating hospital staff on relevant safeguarding matters, completing annual social care reassessments, and facilitating access to welfare rights, housing, advocacy, and other support services. You will work closely with the Safeguarding Lead, managing internal investigations and liaising with home authorities during admission and discharge processes.

• Manage a complex caseload and be responsible for social care assessments.
• Risk assessments.
• Facilitate group activities.
• Contribute to ICR meetings.
• Support service users to make viable plans for their future.
• Report to the Safeguarding Lead, manage internal investigations and liaise with home authorities for service users during admission and discharging process.

As a Social Worker you will be:

• Referrals to LADO and Local Authority in line with safeguarding and assessments
• Educating Hospital staff about relevant safeguarding matters.
• Completing a full annual social care reassessment within the CPA format.
• Facilitating access to welfare rights, housing, advocacy services and associated support services
• Participating in 1:1 supervision

To be successful in this role, you'll need:

• A Social Work degree
• Registration witn Social Work England
• Experience within similarenvironment
• Experience with working withvulnerable adults

Location: Farmfield Drive, Charlwood, Surrey, RH6 0BN

You will be working at Farmfield Hospital, situated located next to Gatwick.

You'll be caring for adult men 18+ who potentially have a forensic background and enduring mental health illnesses such as personality disorder, substance misuse issues and challenging behaviours.

Person Specification Qualifications

• A Social Work degree and registration with Social Work England are required. Relevant experience in a similar environment and working with vulnerable adults is also essential.
