Social Prescribing Link Worker - Community Care Navigator

Arc Bucks Primary Care Network is seeking a Social Prescribing Link Worker to provide one-to-one support, develop personalised care plans and connect patients with community services. You will work within a multidisciplinary team to promote wellbeing and reduce health inequalities.

The role focuses on coaching, motivational interviewing and empowering patients to manage their health, with responsibilities across partnerships, community engagement and continuous improvement.
