Slough Council - Social Worker - Hospital Discharge

Our client requires an experience worker who can think on their feet and discharge within a strict timeline of 3/6 days implementing the D2A model. This requires solid experience and good knowledge/skills to progress a safe discharge within a timely way.

Wexham Park is their main hospital so you will need to be able to travel when required to complete assessments and attend meetings. You will also be expected to work one day on duty based at the hospital and one day at Observatory House. Parking permit can be arranged for hospital parking.

Roles will include safeguarding for all patients that may have experienced neglect/abuse inside the hospital. We are a small supportive team. We use LAS so this would support your application if you have experience of this system.
