This is an exciting opportunity for development in the discharge lounge. We are a dynamic, knowledgeable and adaptable team promoting safe, on the day discharges of all patients, with involvement from many multidisciplinary team members, family and community services.
We are a fast paced environment that works closely together to provide an excellent service, putting the patient at the forefront of any plans.
Enabling an earlier discharge, we are a safety net that allows wards time to care for new clients.
Main duties of the jobTo liaise with the patient, family, care providers, transport teams and many more, both verbally and electronically to ensure a safe discharge, with all follow up care and plans in place. To work independently and as a team to promote this. As a Band 6, lead the team from the front, engaging with team members to enhance their skills and learning, to also liaise with all the wards for engagement and to promote the service. To assist with the daily running of the unit and to stand in for the Band 7 when they are away.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

Job responsibilitiesTo be a clinical role model on shift, providing leadership and guidance with staff well being, morale and the daily running of the unit.
To be involved in daily clinical audits and to be involved in the development of the unit.
To ensure high standards of nursing care, giving quality and knowledgeable information to patients and family on their day of discharge, making sure all care and follow up plans are in place. Recognising the need for adaptability and the initiative to see if patients need further support or guidance.
To take an active role in liaising with other departments promoting the service, and supporting the education and development of others.
Be able to coordinate the service, on a day to day basis, managing complaints and escalating where necessary.
To plan and prepare patients and families on the day of discharge, checking care, follow up plans are in place, medication education has been given, to identify any further needs the patient or the carer may have, being aware of all the community support services available.
Person Specification QUALIFICATIONS / TRAINING- Management Certificate
- Recent Experience at Band 5 within the Critical Care Environment
- Knowledge of a Wide Range of Surgical Specialities
- Evidence of self development/clinical knowledge
- Up to date professional knowledge
- Knowledge of local and national Critical Care strategies and guidelines
- Experience of evidence based practice
- Critical Care Course/Certificate
- Experience in a Band 6 Development Role
- Leadership Course
- Experience of change management
- Proven ability to lead a team
- Excellent written and verbal communication skills
