Senior Mental Health Segregation Nurse

Senior Mental Health Nurse to join the Mental Health team at HMP Leeds, a Category B men's prison housing males aged 21+.
This role works with the Integrated Mental Health Team, focusing on patients within the Segregation Unit. The unit holds up to 21 prisoners and supports those who are disruptive, at risk to themselves or others, or in need of protection.
Hours37.5 hours per week, Monday to Sunday, shift pattern covering days (9.5 hrs). Shifts 8am-6pm. Weekends and bank holidays scheduled throughout the year. No nights.
SalaryFull-time position, annual salary up to £49,175 per annum (FTE), depending on experience.

- Complete regular segregated unit clinical rounds, assessing physical and mental wellbeing, including high risk or vulnerable individuals.
- Carry out initial and ongoing clinical assessments for anyone located within segregation, following NHS and Practice Plus Group clinical pathways.
- Work in partnership with custodial officers, segregation unit staff and the Safer Custody team.
- Support the Primary Care Team to provide emergency care and escalation when clinically required, including urgent GP input and external hospital transfer.
- Operate as part of an integrated health team, working with the Mental Health Team to monitor mood, risk, crisis indicators and treatment adherence for residents with mental health conditions.
- Collaborate with the Complex Case Team to support individuals with multifaceted needs such as personality disorder, neurodiversity, chronic self harm risk, or high levels of distress.
- Balance compassionate, trauma informed clinical practice with the security demands of a busy Category B prison.
- Share clinical observations and attend multidisciplinary reviews to ensure continuity and alignment of physical and mental health care plans.
- NMC Registered.
- Experience working within mental health settings, with knowledge and understanding of this patient group.
- Experience collaborating with other agencies and stakeholders in a multi disciplinary environment.
- Strong Band 6 experience, able to demonstrate leadership skills and work in a high paced environment.
- Experience in a secure forensic setting (low, medium or high).
- Experience in hospital, community or prison environment (ideal but not essential).
All candidates must have a legal right to work in the UK. The role requires additional vetting and security checks. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and a Disclosure and Barring Service check will be required.
