Senior Mental Health Practitioner

We have an exciting opportunity for a Senior Mental Health Practitioner to join our Mental Health team at HMP Stocken, a category C Men's Prison located in Stretton, Rutland.
In this role you will work with a multi disciplinary team that will support you and help you succeed.
The Mental Health Service is covered:
- Monday to Friday 08 00
This is a full time position with an annual salary between £43,609 and £49,175 per annum, depending on experience.

Main duties of the job- Responsible for assessment and ongoing management of patients' mental health needs.
- Work collaboratively with the multi disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan.
- Contribute to our nursing strategy and vision by providing outstanding evidence based care, seeking out, listening to and acting on patient feedback so that care is personalised and informed by what matters to them.
- Deliver non judgemental care, building therapeutic relationships and promoting professional boundaries.
- Play an active part in the assessment, planning, implementation and evaluation of patients' care following our policies and procedures while always considering safeguarding.

Person SpecificationWe are looking for caring, compassionate, and driven professionals who can help us drive our vision for fair and inclusive healthcare access for all.
- Occupational Therapist or Social Worker - HCPC / NMC registered.
- Experience of working within mental health settings, developing knowledge and understanding of this patient group.
- Evidence of continued professional development.
- Experience of working collaboratively with other agencies and stakeholders in a multi disciplinary environment.
- Experience in a hospital, community or prison environment is an advantage.
