We are seeking a skilled and compassionate Senior Mental Health Practitioner to join our dynamic CAMHS Crisis and Home Treatment Team. This is a rewarding opportunity to support children and young people facing acute mental health challenges delivering timely assessments, intensive home-based support, and working closely with families to prevent hospital admissions.

The service operates 7 days a week 8am-8pm, (shifts are 8-8).

The role is open to registered professionals (RMN / Social Worker / Occupational Therapist) with:

  • Experience in CAMHS, crisis, or acute mental health services
  • Strong risk assessment and safety planning skills
  • The ability to work autonomously and within a team
  • A flexible, compassionate, and resilient approach
  • Ability to meet the travel requirements of the role

Main duties of the job
  • Undertake crisis assessments and risk management plans for young people presenting in acute mental health distress
  • Deliver time-limited, goal-focused interventions in community and home settings
  • Collaborate with families, schools, social care, and acute services to provide holistic care
  • Participate in team meetings, case discussions and multi-agency forums
  • Maintain accurate clinical records and contribute to service development initiatives
  • Participate in a 7 day service rota including evenings, weekends and bank holidays
  • Participate in clinical, safeguarding and reflective supervision
  • Work alongside clinical leads and operational leads

Job responsibilities
  • Operate as part of a multidisciplinary team providing Crisis Intervention and Intensive Home Treatment to CYP across Herefordshire.
  • Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to CYP.
  • Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies, families and children and young people.
  • Actively and creatively engage CYP and carers encouraging them to make appropriate use of the services available as part of a process of recovery and crisis management.
  • Undertake full psychosocial assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure.
  • Undertake a acute risk assessment and safety planning for extremely vulnerable client groups during times of crisis.
  • Deliver High quality, evidence-based care to a defined team caseload

Person Specification Experience
  • Extensive post-registration experience in mental health care including mental health assessment and risk assessment
  • Use of therapeutic interventions within mental health settings i.e. 1:1, counselling, family therapy and group work
  • Delivering teaching / training
  • Providing consultation
  • Experience of working in a CYP service or Crisis & Home Treatment service
  • Formulating and communicating a clinical presentation to an MDT
  • Evidence of providing a supervisor role
Qualifications
  • Evidence of post-registered clinical qualifications/training or be able to demonstrate an equivalent level of training to a minimum of postgraduate diploma level
  • Current registration with a relevant professional body
  • I understand that I will be asked to provide original certificates to verify my qualifications at the point of offer.
  • Evidence of ongoing continuing professional development
Skills & Abilities
  • Ability to communicate clearly and effectively in English in both verbal and written formats.
  • Ability to travel to patients at the needs of the service across a wide geographical area in a timely manner
