Parttime post, 18 hours per week working between 17:30-23:30 over 7-day rota.

This role isa band 6 Senior Community Nurse within the Evening & Night team, working 17:30-23:30over 3 evenings a week. The teamis countywide, supporting theSouth of the County, with the flexibility to work countywide as required. Youwill be part of the triage team from the central hub atDroitwich medical centre for some of your shifts, which involves taking callsfrom health professionals and direct calls from patients/relatives, overseeing the electronic referrals that come via EMIS and the westmidlands Ambulance service system CAD. Youwill also have clinical responsibilities and visit patients in their own home andsupport them in remaining at home, preventing admission to hospital andsupporting early discharge from hospital.

The postis all worked as unsocial hours hence picking up pay enhancements for all hoursworked which makes this post more attractive (more money for less hours).

Main duties of the job

As a community Nurseworking with the Evening Team, most of the work allocated to you will of anunplanned nature. The service works as an on-call service covering the Countyof Worcestershire taking direct referrals from patients, 111, Ambulance Serviceand discharging hospitals. You can expect to have a comprehensive induction andwork alongside peers until you are confident to then work alone.

You will primarily work alone, and havework allocated via a handheld tablet by the triage nurse.You will be able to start your visits from home if the geographics dictates. Asa band 6 you will rotate into the triage role based at Droitwich at least once a week. The triage nurse takes the lead for the shift andallocates the work to the team. The county is coveredas 2 parts with a triage nurse for the North and South both based inDroitwich.

The patients seen during the evening and nightvary immensely and no 2 shifts being the same. We see catheters,palliative care, wound care, administration of medication, diabetics, andpatients requiring intensive support rehabilitation following discharge fromhospital or admission prevention. As a team we often undertake initialassessments of patient in the community.

Aspart of a new initiative, we are working closely with West Midlands ambulanceservice and 111 and assist in responding to urgent Care response. These can bepatients that have fallen, or requiring further assessment to aidprevention in admission to hospital.

Job responsibilities

You will bejoining the Evening element of one of our dynamic and supportive NeighbourhoodTeams, to provide a 24-hour admission avoidance service to the population ofWorcestershire which encompasses both urban and rural areas.

Due to thenature of the post the applicant must be able to travel independently and haveaccess to a suitable vehicle for business purposes. Whilst a base will beidentified, staff can be asked to work anywhere within the Business Unit orService if necessary.

Person Specification Qualifications
  • Registered Nurse (RN1)
  • Graduate Nursing Qualification at Academic Level 6 or working towards.
  • Preceptorship training
  • Evidence of on-going continuing professional development
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
  • Accredited courses in palliative care, tissue viability, Diabetes
Experience
  • Significant post registration experience in a clinical setting able to demonstrate transferability of experience to community setting
  • Experience of management of caseloads or wards and organisation of staff
  • Clinical audit/service improvement and development work
  • Experience of active participation in interdisciplinary team working
  • Previous community nursing experience
  • Transcribing medication training
