Senior Clinical Pharmacist- Urgent and Emergency Care

An exciting opportunity has arisen to lead, develop and participate in specialist clinical pharmacy services within the Urgent and Emergency Care service at Gateshead Health NHS Foundation Trust.
We are looking for a dynamic, highly motivated senior clinical pharmacist to provide expert advice on medicines and ensure safe, effective and high quality care to patients within the fast-paced emergency care setting.
Main duties of the jobThe successful candidate will work within the urgent care MDT, providing specialist clinical pharmacy advice to support the patient journey. This includes accident and emergency, same day emergency care and urgent treatment centres.
Key responsibilities include medicines reconciliation, prescribing, supporting administration of time critical medication and facilitating timely discharge. You will work closely with the Emergency care clinical pharmacist to deliver and evaulate a robust clinical pharmacy service across the Emergency Care Centre. You engage with key stakeholders to drive innovation and improve medicines optimisation and provide leadership and line management to pharmacy colleagues.

Job responsibilities- To provide specialist clinical pharmacy input and maintain high standards of medicines management across the urgent and emergency care departments.
- To work closely as part of the MDT within the Emergency Care Centre, including accident and emergency, same day emergency care (SDEC), urgent treatment centre (UTC) and admission wards.
- A focus on supporting the patients journey. This will include ensuring timely medication history and reconciliation, rationalising and prescribing medication, providing patient medication counselling and facilitating safe discharge to ensure patient flow.
- Supporting the prescribing and administration of time critical medication within the emergency department.
- Collaborate with the acute and emergency care pharmacist in relation to service provision and service development within the Emergency Care Centre.
- Support with development and updates of clinical guidelines, policies and SOPs.
- As part of the team you will work closely alongside other pharmacy colleagues, providing leadership, clinical supervision and line management.
- Contribute to innovation in the urgent and emergency care department to look for opportunities for admission avoidance, medicines optimisation and improving transfer of care between hospital and community and front of house to back of house areas.
- Work to deliver the 7 day clinical pharmacy service by supporting the late night, on call and weekend rota.
- Provide support to the lead clinical pharmacist for medicine with regards to medicines management, incident resolving, staff training and integration with the urgent care senior team.
- Engagement with key stakeholders e.g ward managers, matrons, chief matron, service line managers and clinical and safecare leads.
- Clinical pharmacy experience in hospital setting
- Experience of leadership
- Experience of clinical audit or quality improvement
- Experience as a line manager
- Experience of service development or improvement
- Working in urgent and emergency care
- Evidence of in depth clinical pharmacy knowledge
- Excellent written and verbal communication skills
- Specialist knowledge of pharmacy systems
- Evidence of self-reflection and continuous professional development
- IT literate
- Ability to prioritise own workload
- Evidence of innovative practice
- Registered as a pharmacist with General Pharmaceutical Council (GPhC)
- Diploma in Clinical Pharmacy or equivalent experience
