Job Description
Darlington Borough Council is seeking an experienced Senior Practitioner to lead Continuing Healthcare (CHC) within its Adult Social Care service. You will guide CHC processes, support practitioners, and ensure alignment with the National Framework for NHS Continuing Healthcare and the Care Act 2014.
You will identify eligibility, gather robust evidence for Decision Support Tool meetings, and mentor staff to improve outcomes for adults with complex needs, in a fixed-term 18-month role.
