Senior ACP/ANP for Frailty & Care Homes - Flexible Days

NHS is looking for an experienced Advanced Nurse/Clinical Practitioner (ACP/ANP) in Preston. This role involves working within the Primary Care Network to support person-centred care for frail and vulnerable residents in care homes.

You will work collaboratively with multidisciplinary teams to provide advanced clinical assessments and reduce avoidable hospital admissions. The position offers flexible working hours with the potential for additional hours in the future.
