Job Description

Responsibilities

Treat patients who are ill or believe that they might be ill
Perform telephone consultations, telephone triage and online consultation as needed
Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Perform home visits as agreed
Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
Conform to practice procedures for QOF, prescribing incentive schemes, PBC and other enhanced services and directives as stipulated by the National Bodies/ ICB or Tudor Surgery
Complete referral letters as necessary for patients seen
Review and sign repeat prescriptions as and when required
Clinical Governance/Quality Assurance: Follow and contribute to protocols to achieve a high score on the Quality and Outcomes Framework
Enhance current polices and procedures within the practice as required
Ensure data is entered in an efficient and accurate manner on to the practice computer system
Promote confidentiality (in line with Caldicott principles) within the practice
Deal with non-NHS correspondence (such as life insurance, holiday cancellations and work related letters)
Deal with pathology results communications from Out of Hours Service
Ensure that repairs are performed to a high standard and in accordance with Health & Safety guidelines
