Main duties of the job

Diverse Consultation Formats:Face-to-face surgeries, e-consultations, triage, telephone consultations, and home visits.

Autonomous Decision-Making:Lead clinical assessments for undifferentiated and undiagnosed conditions.

Preventive Care Leadership:Conduct disease risk screening and early intervention.

Care Coordination:Develop personalised treatment plans and manage referrals.

Patient Education:Provide counselling and health education that empowers patients.

Quality Assurance:Maintain meticulous clinical records and participate in audit processes.

Digital Integration:Utilise Systm1 for enhanced patient care and data management.

Prescribing Excellence:Follow evidence-based formularies while ensuring patient safety.

QoF & LCS Compliance:Contribute to Quality and Outcomes Framework and Locally Commissioned Services achievements.

Multidisciplinary Teamwork:Collaborate with PCN, system colleagues and healthcare professionals.

Clinical Supervision:Mentor junior doctors, F1/F2/ST trainees, and student placements.

Practice Development:Contribute to policy development and clinical standard setting.

Knowledge Sharing:Participate in practice meetings and educational initiatives.

Were a large practice (circa 19000 patient population) based in the heart of the B&NES countryside. We have a large multidisciplinary team, supported by a range of clinical modalities including nursing, a pharmacy team, physiotherapy and home visiting practitioner.
