Main duties of the jobDiverse Consultation Formats:Face-to-face surgeries, e-consultations, triage, telephone consultations, and home visits.
Autonomous Decision-Making:Lead clinical assessments for undifferentiated and undiagnosed conditions.
Preventive Care Leadership:Conduct disease risk screening and early intervention.
Care Coordination:Develop personalised treatment plans and manage referrals.
Patient Education:Provide counselling and health education that empowers patients.
Quality Assurance:Maintain meticulous clinical records and participate in audit processes.
Digital Integration:Utilise Systm1 for enhanced patient care and data management.
Prescribing Excellence:Follow evidence-based formularies while ensuring patient safety.
QoF & LCS Compliance:Contribute to Quality and Outcomes Framework and Locally Commissioned Services achievements.
Multidisciplinary Teamwork:Collaborate with PCN, system colleagues and healthcare professionals.
