Job Description

You will manage a caseload and deal with a wide range of health needs in a Primary care setting, ensuring the highest standards of care for all registered and temporary patients

Main duties of the job

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working in primary care;

surgery consultations, telephone consultations, queries, home visits, repeat prescriptions, dealing with these in a timely fashion.

Assessing healthcare needs of patients with undifferentiated and undiagnosed problems

Promoting and maintaining health and safety for their own and other members of staff

JobSummary:

The post-holder will manage acaseload and deal with a wide range of health needs in a primary care setting,ensuring the highest standards of care for all registered and temporarypatients.

ClinicalResponsibilities:

Inaccordance with the Practice timetable, as agreed, the post-holder will makehim/her-self available to undertake a variety of duties including surgeryconsultations, telephone consultations and queries, visiting patients at home,checking and signing repeat prescriptions and dealing with queries, paperworkand correspondence in a timely fashion

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation

Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems

Screeningpatients for disease risk factors and early signs of illness

Inconsultation with patients and in line with current Practice disease managementprotocols, developing care plans for health

Providingcounselling and health education

Admittingor discharging patients to and from the caseload and referring to other careproviders as appropriate

Recordingclear and contemporaneous consultation notes to agreed standards

Collectingdata for audit purposes

Compilingand issuing computer-generated acute and repeat prescriptions (avoidinghand-written prescriptions whenever possible)

Prescribingin accordance with the Practice prescribing formulary (or generically) wheneverthis is clinically appropriate

Ingeneral the post-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

Awarenessof and compliance with all relevant Practice policies/guidelines, e.g.prescribing, confidentiality, data protection, health and safety

Acommitment to life-long learning and audit to ensure evidence-based bestpractice

Contributingto evaluation/audit and clinical standard setting within the organisation

Contributingto the development of computer-based patient records

Contributingto the summarising of patient records and Read-Coding patient data

Attendingtraining and events organised by the Practice or other agencies, whereappropriate.

In the performance of the dutiesoutlined in this Job Description, the post-holder may have access toconfidential information relating to patients and their carers, Practice staffand other healthcare workers. They may also have access to informationrelating to the Practice as a business organisation. All such informationfrom any source is to be regarded as strictly confidential

Information relating to patients,carers, colleagues, other healthcare workers or the business of the Practicemay only be divulged to authorised persons in accordance with the Practicepolicies and procedures relating to confidentiality and the protection ofpersonal and sensitive data.
