Full-Time Support Worker (Surrey and the South East)

Assisting young people in the transition from adolescence to adulthood and helping them to live independently.

• Assist young people to live independently.
• Promote the five outcomes in the Every Child Matters Green Paper.
• Provide a safe, caring, and supportive environment for young people.
• Assist young people in the transition from adolescence to adulthood by equipping them with life skills and empowering them.
• Help young people sustain their accommodation, providing mediation when required.
• Collaborate with the young person and all significant persons in designing and implementing care plans, taking into account the young person's views.
• Assist young people to live independently within a shared house.
• Monitor the safety and wellbeing of the residents.
