Overview

Renal Young Adult Youth Worker - Northern Care Alliance NHS Foundation Trust. This is a one-year part time (7.5 hours), Band 5 post for a Youth Worker (YW) to support the Renal Young Adult Clinic.

This is a one-year part time (7.5 hours), Band 5 post for a Youth Worker (YW) to support the Renal Young Adult Clinic.

Main duties of the job
  • To participate as a professional member of the healthcare team and contribute to the care of the patients as part of the multidisciplinary team.
  • To act as a social prescribing link worker and enable access to local community services for appropriate recreational, social, educational, vocational and advocacy support where appropriate/on request of young person.
  • To contribute to the provision of a varied programme of activities and opportunities to engage young people, and support engagement with the national Young Adult Kidney Group.
  • To support young people transitioning from Paediatric to Adult Care.
