Job Description

Job Title: Clinical Pharmacist (Primary Care) Location: London (Remote) Salary: £46,000 to £53,000 (depending on experience) Working Hours: Mon, Tues, Wed between 8AM-6PM - 7.5 hours per day

The Role

As a Clinical Pharmacist, you will be an integral part of our remote clinical team, providing best-in-class medication management, medicines optimisation, and proactive patient care. You will also play a key role in implementing new initiatives and driving innovation across our GP practices.

Key Responsibilities
  • Deliver high-quality, patient-centred pharmaceutical care, improving health outcomes for all registered patients.
  • Work collaboratively with the wider multidisciplinary team (MDT) to implement innovative solutions that enhance patient care and clinical efficiency.
  • Handle prescription and medication queries efficiently and safely.
  • Conduct structured medication reviews to optimise patient treatment plans.
  • Monitor and manage repeat prescriptions to ensure safe prescribing practices.
  • Perform medicines reconciliation for patients transitioning between care settings.
  • Support chronic disease management, enhancing treatment plans for long-term conditions.
  • Lead and contribute to quality improvement projects to advance patient care and clinical excellence.
  • Foster a collaborative, ambitious, and innovative culture, helping to build the best primary care service in the UK.
  • Communicate effectively with patients, carers, and colleagues, ensuring professionalism, efficiency, and clarity in all interactions.
  • Work within a supportive clinical environment, with access to senior clinicians and GPs for mentorship and professional development.
Who We're Looking For

We are seeking a proactive, innovative, and highly organised pharmacist who thrives in a fast-paced, patient-focused environment.

Essential Requirements:

  • Registered Pharmacist with professional registration with the General Pharmaceutical Council (GPhC).
  • Independent Prescriber qualification.
  • Experience in chronic disease management and prescribing.
  • Commitment to continuous professional development, with evidence of ongoing training and education.
  • Post-registration experience in primary care.

Desirable Qualifications & Experience:

  • MSc in Advanced Clinical Practice.
  • Completion of the Primary care pharmacy education pathway
  • Experience in managing and prescribing for minor acute conditions.
