Job Description
Registered Nurse Hospice at Home- Mansfield We are expanding our Hospice at Home service and are seekingcompassionate, committed people to join our growing team. This role offers theopportunity to support patients and families in their own homes as the serviceextends across North Nottinghamshire.

The Hospice at Home service provides care for palliative patientsseven days a week in their own home.

This role will be based in Mansfield with travel across North Nottinghamshire to meet organisational needs. The specific base locations within this area have yet to be confirmed.

Main duties of the job As a Registered Nurse within the Hospice at Home service, you willdeliver high-quality, person-centred nursing care to patients in their ownhomes across North Nottinghamshire. You will support patients with palliativeand end of life care needs and provide appropriate support to families andcarers, in line with hospice policies and agreed care plans.

The role includes assessment, planning, and delivery of nursingcare, symptom management, care of the dying, and psychosocial and spiritualsupport. You will promote independence where possible, support discharge home,and respond flexibly to changing patient needs.

You will work as an autonomous practitioner while remaining partof a supportive multidisciplinary team. You will work closely with the Hospiceat Home Lead, Health Care Assistants, Care Navigators, the Integrated Hub, andwider system partners within the End-of-Life Care Together Alliance.

You will contribute to maintaining high standards of care, safety,and quality, ensuring practice aligns with NMC standards, CQC requirements, andthe values and philosophy of Beaumond House Hospice Care.

BeaumondHouse Hospice Care is a registered charity. We provide four In-patient beds,Day Therapy, and Hospice at Home Care. Hospice at Home supports people withpalliative and end of life care needs, often in the last weeks and months oflife. Our nurses and health care assistants provide assessment, personal care,emotional support, advice, and advance care planning in the persons preferredplace of care.

We arecontactable 24 hours a day, 7 days per week, to provide a friendly ear, generalinformation or to signpost patients, relatives and carers to appropriateresources and services.

Hours: Part-time and full-time opportunities available (job shares considered)

Salary: £19.10 per hour

Location:Mansfield, with travel across North Nottinghamshire to meet organisational needs.

Main Duties

Integrated Working and Care Coordination
  • Work in partnership with the Integrated Hub, Hospice at Home Lead and Care Navigators.
  • Support discharge planning and reduce avoidable hospital admissions.
  • Communicate effectively with patients, carers and professionals.
  • Attend and contribute to MDT, GSF and Hub meetings.
  • Build strong working relationships with partner organisations.
Leadership, Supervision and Workforce
  • Lead and support Health Care Assistants in delivery of care
  • Delegate duties appropriately in line with NMC guidance.
  • Act as role model, supervisor and mentor.
  • Contribute to recruitment, induction and staff development.
  • Monitor staffing levels and escalate concerns as required.
  • Support students and learners in practice.
Safeguarding, Risk and Governance
  • Act in accordance with safeguarding adults policies.
  • Undertake risk assessments including lone working.
  • Maintain accurate documentation in line with NMC and CQC standards.
  • Comply with medicines management policies.
  • Participate in audits, incident reporting and quality improvement.
Bereavement Support
  • Provide bereavement support to carers and families.
  • Identify and refer those requiring specialist bereavement services.
Professional Development
  • Maintain NMC registration and comply with the NMC Code.
  • Engage in reflective practice, supervision and appraisal.
  • Attend mandatory and developmental training.
  • Build resilience and support emotional wellbeing.
Skills, Experience and Attributes Required
  • Experience in caring for palliative patients or relevant transferrable experience.
  • Good telephone and interpersonal communication skills.
  • Ability to work as part of a team and respect for all individuals.
  • Flexibility, excellent listening skills and ability to build rapport with others, with a friendly and sensitive approach.
  • Confidence in use of basic IT (Microsoft 365) and electronic records (SystmOne) or willingness to develop this.
  • Full driving licence and regular access to a suitable vehicle.
  • Ability to communicate effectively in spoken and written English to support safe care, accurate record keeping, and sensitive communication with patients and families.

  • Work collaboratively and cooperatively with Beaumond House colleagues and End of Life Care Together Alliance partners.
  • Work in accordance with hospice values of compassion, dignity, respect and excellence, recognising the emotional impact of end-of-life care.
  • Respect confidentiality in all areas of Beaumond House.
  • Comply with the Health and Safety at Work Act.
  • Work within Beaumond House policies and procedures.
  • Participate in and contribute to team meetings and Hub meetings as required.
  • Always behave in a professional manner, reflecting and maintaining Beaumond House values.
