Job Description

Do you want to make a real difference every day at work?Are you the right person to complete our Hospice at Home team?

The Hospice at Home Team support people who wish to spend their last days in their own home. We provide symptom control and psychological and emotional support to not only the patient but also to their loved ones who are caring for them. We are looking for a committed registered nurse to join our team. With support from the Hospice at Home Sister, you would be responsible for planning and managing care for patients wishing to die at home.

Main duties of the job

What youll bring

To be successful in this role you will need to be a registered nurse with a passion for palliative care, have excellent communication skills, and a desire to work in the community. Community and/or palliative care experience would be an advantage.

Asa Registered Nurse you will deliver this by providing professional care andsupport to our patients and families in their own homes, working alongside aHospice at Home Sister/Charge Nurse.

Key roles and responsibilities:

  • Provide specialist nursing care topatients in their own homes. Carry outassessments, arrange care plans, monitor symptoms and adjust medication withthe guidance of the senior nurse on duty and working closely with HCAs andSenior HCAs, provide personal care to patients.
  • Liaise with other communityagencies to provide the best care.
  • Provide emotional support tofamilies.
  • Verify deaths and provide careafter death with the support of the Healthcare Assistants and Senior HealthcareAssistants.
  • Work across the 24/7 serviceprovision.
  • Promotea culture of continuous learning and development and wellbeing, help to create an environmentthat is continually critically questioning practice and promoting learning.
  • Identify your own learning anddevelopment needs undertake continuous professional development and activelyengage in clinical supervision, maintaining an objective and innovativepractice.
  • Provideeducation both internally and externally. Provide mentoring and support tojunior members of the team and students.
  • Playan active part in team meetings and ensure your mandatory and other relevanttraining is kept up to date.

Infection Prevention

All Rowcroft employees are required to adhere totheInfection Prevention and Control Policies and make every effort tomaintain high standards of infection control at all times to reduce the burdenof Healthcare Associated Infections, including MRSA.

You agree to the following:

  • To decontaminate your hands prior to and afterdirect patient care or contact with the patient's surroundings;
  • To take part in mandatory infection controltraining provided;
  • To responsibly manage your own infections.

Placeof Work

This role has been identified as aField worker the role could be carried out at a Rowcroft site or otherlocation subject to the varying needs of the role. Will spend planned time at Rowcroft sites orother locations as the role requires which could include patient homes andRowcroft and non-Rowcroft locations to meet the needs of the service.

This will be fully discussed and agreed with thepostholder to meet individual and business needs.

Whatis Hospice at Home?

Hospiceat Home cares for patients that want to be able to die in their own homes (orcare home) and are thought to be in their last 2 weeks of life.

We are ateam of registered nurses and healthcare assistants that prioritise what isimportant to the patient and those closest to them, working with otherhealthcare professionals in order to achieve the patients last wishes.

We wereawarded the Choice and Control award for personalised care in 2022 inrecognition for offering patients choice over where they wanted to die andgiving them some control at the end of their life.

Person Specification Personal Requirements
  • Access to transport and able to work on your own in peoples homes.
  • Friendly, welcoming and inclusive.
  • Leadership through creative problem solving, a positive can-do attitude and a willingness and desire to ensure all who come into contact with Rowcroft have the best possible experience.
  • A desire to be part of a committed and hardworking team in line with the ethics and values of Rowcroft Hospice, acting in the best interests of Rowcroft at all times.
  • A commitment to continually learn and develop.
  • Willing and able to volunteer for at least one Rowcroft event each year.
  • A good understanding of the purpose, values and strategy of Rowcroft Hospice
Qualifications
  • Registered Nurse on Part One (Adult) of the register.
  • Relevant qualification in palliative care/Care of the Dying or qualification through experience.
Experience
  • Proven experience of assessment and care planning.
  • Proven experience of contributing effectively within a Multi-disciplinary team.
  • Medicine management, including syringe pumps.
  • Counselling course.
Knowledge and Skills
  • Proven assessment and care planning skills.
  • Sensitivity and an insight into the needs of patients and their families.
  • Full driving licence.
