We're looking for a Recovery Coordinator to join our Leamington Spa team, with outreach work across Stratford-upon-Avon.
You'll support people affected by their own drug or alcohol use, as well as those concerned about someone they care about. Through honest conversations, personalised planning and joined-up support, you'll help people make positive and lasting changes.

As someone's main care coordinator, you'll take time to understand their strengths, needs and goals. You'll complete assessments, develop individual recovery plans and bring together the clinical, psychosocial and community support that can help them move forward.
Working alongside colleagues and partner organisations, you'll help make treatment accessible, responsive and centred around what matters to each individual.
What you'll be doingYou'll:
- Complete assessments, recovery plans, risk assessments and regular care reviews.
- Coordinate personalised treatment and support, including clinical, psychosocial and harm-reduction interventions.
- Build trusted relationships with people using the service and those supporting them.
- Work closely with health services, mental health teams, GPs and community partners.
- Support safeguarding, reduce drug and alcohol-related harm and make appropriate referrals.
- Help people connect with peer support, mutual aid and wider recovery opportunities.
- Maintain accurate, timely and confidential records.
Location: You'll work as part of our Leamington Spa team and provide outreach support across Stratford-upon-Avon. The ability to travel between locations and work within community settings is required.
Hours: Full time, 37.5 hours per week.
Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and any associated payments will be pro rata based on contracted hours.

You'll be able to build trusting relationships, have open conversations about risk and change, and remain calm when situations are complex.
You'll bring:
- A strong understanding of drug and alcohol use, with experience in a related field.
- Experience completing assessments, managing risk and coordinating support.
- Strong communication, organisation and time-management skills.
- The ability to work independently while remaining a collaborative team member.
- A commitment to partnership working and person-centred care.
- Good IT skills and the ability to maintain accurate records.
Most importantly, you'll believe in people and their ability to make positive changes.
