• Developing and implementing care plans in collaboration with clients and relevant stakeholders.
• Providing ongoing support and monitoring progress, adjusting care plans as necessary.
• Advocating for clients' rights and access to services, ensuring they receive the support they deserve.
• Maintaining accurate and timely documentation in compliance with regulatory standards.

Essential qualifications and skills

• A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Registration with Social Work England.
• Demonstrated experience in adult social care, preferably in a community setting.
• Strong assessment and analytical skills, with the ability to make sound judgments.
• Excellent communication and interpersonal skills, with a compassionate approach to client care.
