Main duties of the jobYou will be a core member of the Product Delivery Team, with a service base of Exchange House, but working independently and flexibly from rented clinic rooms, office spaces, your home and visiting patients in their home if needed all around Kent. Appointments are face to face, virtual on video or by telephone.
You will be clinically assessing patients for their eligibility of continence products, aiming to ensure that patients can manage their continence whilst maintaining their dignity. You'll also be key in supporting and educating patients, family, carers and care homes with their own or their family/patient's self care and product usage.
Additional Working Pattern: 9am - 5pm Monday - Friday.
Driving requirementAs a community Trust, many of our roles require individuals to be able to drive to perform their duties. Where driving is a requirement of the role, you will need to confirm that you hold a full UK driving licence or a full driving licence issued by an EU country (not exchanged from a non EU country). Person Specification Qualifications, training and experience- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
- You'll need to have recent experience of healthcare gained through education, work experience or previous employment.
- You will be a registered health care professional with the Nursing and Midwifery Council.
- A full, valid UK driving licence or a full driving licence issued by an EU country.
- You will need to have a customer focussed approach which is essential to provide patients, clients and their families with the best possible experience.
- You will need to be able to use IT systems and databases.
- Having an awareness of developments within the NHS would be an advantage.
- You will need to understand where to signpost clients and families to alternative services to support behaviour change and improve health.
- You will have knowledge of up to date clinical practice, using up to date skills and adhering to evidence based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary£32,073 to £39,043 a year pro rata per annum.
