Practice Nurse (PCN Employed) The closing date is 24 April 2026

This is a Band 5 registered nursing role supporting GPs and the wider multidisciplinary team in the delivery of general practice services. The post holder will work autonomously within their professional scope of practice, providing evidence based nursing care, health promotion and chronic disease management in line with PCN and practice protocols.

The role is part time, 30 hours per week over 4 or 5 working days. Some flexibility in working days or hours may be considered for the right candidate. As part of the PCN's Enhanced Access service, participation in Saturday clinics is required on a pro rata basis, typically around one in eight Saturdays.

Main duties of the job The post holder will undertake the following duties in accordance with professional standards, PCN and practice protocols, and service need:

Clinical Duties
  • Dressing and removal of sutures
  • Primary and secondary prevention of CVD
  • Travel Immunisations and Vaccinations
  • Cervical smears
  • Contraception; uncomplicated Pill checks, injections
  • Electrocardiograms (ECGs)
  • Urinalysis
  • Long term conditions (Diabetes, Hypertension, Chronic Respiratory Illness etc)
  • Adhere to code of professional conduct as stipulated by governing body
  • Home Visits as per Practice/Operational needs
Research Projects
  • Co operate and support research undertaken by the Practice including the referral of suitable patients who may benefit as participants in research programmes
  • Student/Staff Education & Training
  • Participate in the induction, education and training of all new members of practice staff including students of all disciplines where appropriate
  • Maintain effective liaison with other healthcare teams concerned with patient care as appropriate and with all other disciplines within the practice, exercising regard for patient confidentiality at all time
About us Healthier West Wirral Primary Care Network (PCN) is seeking a motivated and compassionate Practice Nurse to join our multidisciplinary team.

While the PCN will employ the post holder, they will be aligned with Myrtle Group Practice and fully embedded within the practice nursing team, working closely with practice staff and clinicians to deliver high quality, patient centred care.

Myrtle Group Practice was formed on 1 May 2024 following the successful merger of Heswall & Pensby Group Practice and Commonfield Road Surgery. Both sites have served their local population for over 40 years and together support a registered list of approximately 22,500 patients. The practice maintains high QOF achievement and is rated Good in all areas by the CQC.

Job responsibilities Main Duties and Responsibilities

The post holder will undertake the following duties in accordance with professional standards, PCN and practice protocols, and service need:

Supplies & Equipment
  • Ensure that equipment is used and stored as manufacturers recommendation
  • Demonstrate awareness of due dates on equipment requiring calibration and/or maintenance
  • Report any equipment that does not fulfil the manufacturer's operating criteria to the Lead practice nurse and/or maintenance person
  • Liaise with Nursing Team when stores require ordering
  • Ensure that all stock is stored correctly as per COSHE regulations
  • Pathological Specimens & Investigatory Procedures
  • Undertake the collection of pathological specimens including intravenous blood samples (when required), swabs, cervical smears
  • All procedures requiring collection of pathological specimens are performed within agreed Practice protocols and in accordance with the Health & Safety Executive
  • Adhering to IPC Guidelines
  • Administrative & Professional Responsibilities
  • Participate in the administrative and professional responsibilities of the practice team.
  • Ensure that all patient information following consultations, registration, and/or including telephone encounters are recorded accurately on the computer, using EMIS and READ CODE as appropriate
  • Attend and participate in practice nurse meetings as able.
  • Assist in the formulation of practice philosophy, strategy and policy
  • To participate in the formulation of practice protocols in conjunction with the SHA and/or WICCG.
  • Undertake and participate in Practice protocols
Professional Development
  • Maintain continued education by attendance at courses and study day as deemed useful or necessary for professional development. Ensure PIN number is always valid
  • Receive clinical supervision to further develop reflective skills resulting in increased clinical effectiveness in practice (dependent on organisational need)
  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
  • Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
  • Ensure mandatory eLearning modules completed in a timely manner when due

Additional Information The post holder is required to comply with all relevant policies and procedures pertinent to their post. Failure to follow correct policies and procedures may result in disciplinary action.

In Accordance with the Risk Management Strategy, employees will participate, whenever required, with the risk management process. They will support line managers by attending mandatory and statutory training, completing incident/accident forms for every adverse event or near miss that occurs, report all defects and complaints, and communicate any dangerous situation to individuals potentially at risk.

By continually re examining our processes, we will be able to develop and improve the overall effectiveness of the way we work.

The responsibility for this rests with everyone working within the PCN and practices to look for opportunities to improve quality and share good practice and to discuss, highlight and work with the team to create opportunities to improve patient care.
