Practice Nurse: Chronic Disease Care & Patient Education

Provide evidence based nursing care across long term condition management, including Asthma, COPD, Diabetes, Hypertension, and Cardiovascular disease.

Supporting patients in self management of their conditions through education and action plans.

Performing blood pressure monitoring, FENO and spirometry to assess and track disease progression.

Undertake cervical cytology, childhood and adult immunisations, travel health advice, and vaccinations.

Perform wound care, dressings, suture removal, and other treatment room procedures.

Support patients with lifestyle advice, health promotion, and self management strategies.

Offer guidance on contraception, STI testing, and reproductive health.
