Job Description

An excellent opportunity has arisen for a Practice Nurse to join our friendly and established nurse team.

The post holder will playa vital role in delivering high-quality, patient-centred care within our Practice, with a particular focus on the management of long-term conditions.

We are looking to recruit for 20 hours per week (negotiable).

Main duties of the job

The post holder will deliver high quality, evidence based practice nursing services as an autonomous practitioner, with a lead role in long term conditions, working within the multidisciplinary team to meet the needs of the practice population.

Job responsibilities
  • Deliver a high standard of nursing care within a general practice setting, ensuring patient safety and well-being
  • Provide high-quality, evidence-based care for patients with long-term conditions such as diabetes, asthma, COPD, hypertension, and cardiovascular disease.
  • Support the management of long-term conditions (e.g. diabetes, asthma, COPD, hypertension) through routine monitoring and reviews
  • Undertake a range of clinical procedures, including wound care, dressings, suture removal etc.
  • Carry out immunisations and vaccinations in line with national schedules and guidelines.
  • Provide health promotion advice on areas such as smoking cessation, weight management, diet, and exercise.
  • Assist with infection prevention and control, including maintaining a clean and safe clinical environment.
  • Manage and maintain clinical stock, including ordering and safe storage of vaccines and medications.
  • Record accurate and up-to-date patient information using electronic health record systems.
  • Work collaboratively with GPs and the wider multidisciplinary team to deliver coordinated care.
  • Recognise and respond appropriately to safeguarding concerns for children and vulnerable adults.
  • Participate in audits, quality improvement activities, and practice development initiatives.
  • Maintain professional development and ensure practice is in line with current clinical guidelines and standards.
Person Specification Experience
  • Broad knowledge of general practice and experience of working as a Practice Nurse
  • Experience with chronic disease management
  • Experience of working autonomously
  • Experience of infection prevention and control measures
  • Experience of quality initiatives, i.e., bench marking
  • An appreciation of the NHS landscape including the relationship between individual organisations, PCNs and the commissioners
Knowledge and Skills
  • Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and womans health (cervical cytology, contraception etc.)
  • Request pathology tests and process the results, advising patients accordingly
  • Understanding of the importance of evidence-based practice
  • Ability to promote best practice regarding nursing matters
  • Ability to work within own scope of practice and understand when to refer to GPs
  • Good clinical system IT knowledge and an ability to record accurate clinical notes
  • Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
  • Understanding of safeguarding adults and children
  • Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
  • Effectively able to communicate and understand the needs of the patient
  • Broad knowledge of clinical governance
  • Knowledge of public health issues in the local area
  • Awareness of issues within the wider health arena and knowledge of health promotion strategies
  • Knowledge of national standards that inform general practice (NSF NICE guidelines)
Qualifications
  • Registered Nurse (Nursing and Midwifery Council)
  • Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
