Job Description

We are looking for an experienced Practice Nurse to join our large team of ANPs, Nurses and Health Care Assistants by working across our two sites.

You will work as part of the Nursing team, delivering chronic disease management while also responsible for childhood immunisations, cervical screening and travel vaccinations to name a few areas. You will bring ideas for quality improvement and streamline nursing services, and a passion for outstanding patient experience.

You will report to and work closely with our Practice Manager and Clinical Team to support our patients in the best way possible.

Main duties of the job
  • ECGs
  • Venepuncture
  • New patient medicals
  • Chaperone procedure
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Diabetes
  • Asthma
  • COPD
  • CHD
  • Immunisations (routine, childhood and travel)
  • Understand the importance of evidence based practice
  • Broad knowledge of clinical governance
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understand when to refer to GPs
  • Understanding of infection prevention and control measures
  • Knowledge of health promotion strategies
  • Understand the requirement for PGDs and associated policy

Depending on experience, we will encourage you to support the training and development for nurses and health care assistants within the Practice. The successful applicant along with the practice manager will contribute to continuous development of nursing services to deliver efficient, effective and outstanding patient services.

Job responsibilities
  • To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being
  • To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
  • To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • To request pathology services as necessary
  • To maintain accurate clinical records in conjunction with extant legislation
  • To ensure read codes are used effectively
  • To chaperone patients where necessary
  • To prioritise health issues and intervene appropriately
  • To support the team in dealing with clinical emergencies
  • To help with supervision and support our HCAs
  • To support the clinical team with all safeguarding matters in accordance with local and national policies
  • To deliver opportunistic health promotion where appropriate
  • To be our practice Infection Control Lead with help from other members of the team
Person Specification Qualifications
  • RGN Registered
  • Post Graduate Diploma or Degree (Chronic Disease Management)
Experience
  • Experience of working within a nursing team
  • Experience in General Practice

  • Cervical Smears
  • Childhood Immunisations
