Main duties of the job- Chronic Disease Management:
- Provide management and monitoring of long-term conditions.
- Provide personalised care plans to support patients in managing their health.
- Undertake annual reviews, medication monitoring, and lifestyle advice to promote improved outcomes.
- Work closely with GPs and other healthcare professionals to deliver integrated care.
- Clinical Duties:
- Administer immunisations and vaccinations.
- Conduct cervical smears, wound care, and health screening.
- Provide health promotion advice and education to support healthier lifestyles.
- Teamwork and Collaboration:
- Work as part of a friendly, supportive team to provide the practice with a high level of nursing care.
- Participate in multidisciplinary meetings and contribute to quality improvement initiatives.
- Mentor and support junior staff or trainees as required.
- Deliver health promotion and lifestyle advice to help patients make positive health choices.
- Actively participate in multidisciplinary team meetings and quality improvement initiatives.
- Contribute to the training and mentoring of junior staff or trainees.
Woodbridge Hill Surgery is a well-established and welcoming GP practice located in the heart of the Guildford community. Our patient-focused approach is underpinned by a team of dedicated healthcare professionals working collaboratively to deliver excellent care.

The role is varied and covers all aspects of practice nursing including cervical cytology, adult and child immunisations, travel vaccinations, chronic disease management, wound care management, health screening, health promotion, ear care, ECG's, ABPIs, venepuncture, INR monitoring, infection control, emergency management anaphylaxis and resuscitation techniques, maintaining patient records and working within NMC guidelines.
We would ideally be looking for experience of working in GP surgery.
Person Specification Qualifications- Qualified RGN/RN
- NMC registered
- Active NMC registration.
- Competence in delivering evidence-based, nurse-led clinics.
- Excellent communication, organisational, and teamwork skills.
- A passionate, proactive and compassionate approach to patient care.
- Significant experience in chronic disease management, particularly diabetes and respiratory conditions.
