North House Surgery, located in Crook, has an exciting opportunity to expand our team of Practice Nurses by recruiting another Practice Nurse to join our friendly and supportive team at North House Surgery.
North House Surgery is a friendly Practice with an approximate list size of 13,500 patients. We are a busy Practice situated in the heart of Crook. We are looking forward to welcoming a Practice Nurse to expand and enhance our team, working part time.
As well as general practice nursing duties (including, but not exclusive to, women's health and screening, vaccinations, injections and health promotion), this post has a substantial focus on delivering effective individualised care to patients.
The successful candidate will be required to work within the professional boundaries of a qualified nurse and provide an extremely high standard of nursing care for patients registered at the practice.
Key responsibilities- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and wellbeing programmes
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
- To manage patients with chronic disease (e.g. respiratory reviews, diabetes reviews)
- To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- To prescribe medication for patients in accordance with PGDs and own scope of practice
- To provide wound care / wound management
- To request pathology services and process pathology results as required
- To maintain accurate clinical records in conjunction with extant legislation
- To ensure read codes are used effectively by all members of the nursing team
- To ensure compliance with the practice chaperone policy
- To assist GPs with minor surgery when required
- To prioritise health issues and intervene appropriately
- To support the team in dealing with clinical emergencies
- To recognise, assess and refer patients presenting with mental health needs
- To administer adult and child vaccinations
- Women's Health And Screening
- To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- To deliver opportunistic health promotion where appropriate
- To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
- To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
- To support the clinical team with all safeguarding matters, in accordance with local and national policies
- Develop and review audit protocols and process for the practice, effectively utilising the audit cycle
- Develop, review and implement nursing protocols in conjunction with the partners
- Support all members of the nursing team, providing guidance when necessary, participating in appraisals and training needs analyses as required
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the practice in order to improve patient care
- Continually review clinical practices, responding to national policies and initiatives where appropriate
- Participate in the review of significant and near miss events applying a structured approach, i.e., root cause analysis (RCA)
Our current clinical team consists of 7 GP Partners, 2 Salaried GPs, 2 ANPs and 3 GP Registrars.
Our surgery is housed in purpose-built accommodation located in the semi rural town of Crook. We also serve the surrounding villages of Howden le Wear and Witton le Wear.
North House Surgery is a well-established training practice for both GP Registrars and Medical Students. We have a strong focus on education and run regular in house teaching sessions.
We have a fantastic nursing team including 5 Practice Nurses, 4 Health Care Assistants, and a Phlebotomist. North House Surgery is also well supported by Pharmacists, Pharmacy Technicians, Social Prescribers and Wellbeing Coach.
Our CQC inspection was very positive with us gaining an overall score of Good. The Practice is actively involved in our local PCN and Federation.
Our clinicians are supported by an excellent administrative team led by our experienced Practice Manager. They work hard to ensure the Practice runs smoothly. North House Surgery is a GMS practice with a list size of 13,500. We are offering a salary depending on experience, CQC Good rating from 2018 High QOF achievers, SystmOne practice. We're a friendly bunch here at North House Surgery and we're looking for a like-minded Practice Nurse who would be willing to help the Practice to continue to provide a high level of care.
Person Specification Qualifications- Registered Nurse (Nursing and Midwifery Council)
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- New patient medicals
- Requesting pathology tests and processing the results, advising patients accordingly
- Understanding the importance of evidence-based practice
- Understand the requirement for PGDs and associated policy
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understanding when to refer to GPs
- Chaperone procedure
- Womens health (cervical cytology)
